You must make an appointment with Boston South Shore Natural Therapies in order to begin the registration process. We will evaluate you to determine if you are qualified for the medical use of marijuana. If you are qualified, we will issue you a certification through the MMJ Online System.

    To bring with you:

    • New Patients :
      • Medical Record
        The record do not have to be extensive, but should satisfactorily document the medical condition for which you wish to use medical marijuana.
      • Picture Identification
        Driver’s license or Picture ID with proof of residency (e.g.: utility bill in your name).
      • A copy of your social security card
      • BSSNT Intake Form (Download PDF and Print)
    • Renewals:
      • If your condition has been stable since your last visit, and your certification expired less than six months ago, no new medical records are necessary. If your medication or treatment has changed, please bring a copy of  the progress note from your last visit with your PCP or specialist.
      • Picture Identification

    To make an appointment you must be at least 18 years of age. If you are under age, please contact our office for specific instructions.

    After you obtain the registration number from Boston South Shore Natural Therapies, it is necessary to then register online with the Medical Use of Marijuana Program.

    Registering on-line is the fastest and most convenient way to register to possess marijuana for medical purposes.
